I just read an article which makes a very convincing argument that the video released by Uber was actually way underexposed. The area was actually very well-lit and the victim didn’t actually “come out from the shadows.” The driver should have had no problem stopping in time.

I’m pretty sure I read that the shooter entered the school through a back entrance. What good is an armed guard if there are other ways to get in?

I honestly don’t understand what you just said, but I’ll spell it out for you anyway:You are not allowed to say Hashem’s name with your head uncovered. The M”B in 91:10 says that your shirt sleeve is an acceptable covering for this purpose.
The M”B in 91:12 says that for davening, one must wear a hat like he would wear when walking in the street or meeting with an important person, and that the small hat that is commonly worn under one’s hat is not sufficient (some have interpreted this to mean that two coverings are required, but as I stated before, I don’t hear it).
Just look in the Mishnah Berurah. This is all very clear.

I didn’t make up that funny example myself. I saw it in the Mishnah Berurah. The Mishnah Berurah uses that example regarding the issue of covering your head for mentioning Hashem’s name in general, not
specifically for bentching or davening. It’s clearly two separate issues.I know these things because I actually looked in the Mishnah Berurah, rather than listing sources that I saw in a footnote or forum somewhere without seeing it inside.

Eh. Based on the context, I would say that’s a weak proof. He just means not the small kova that is commonly worn under a hat. If there was a specific inyan of two coverings, he would have said it straight out.
I’ve seen rabbonim take off their yarmulkes as they put on their hats for davening.
