(This should have been in my last post nut it was already long enough.)
R’ Herzog emphatically rejects the Ohr Sameach’s pshat. He agrees with your sensibilities (Randomex) that it doesn’t make sense that the murderer can’t leave for pikuach nefesh, but he concludes that it’s a gezeiras hakasuv and ‘???? ??????? ?????? ????????? ?????? ????? ??????? ? (he doesn’t quote the entire pasuk). So according to him Dorothy would not have been able to leave under any circumstances, barring the death of the kohen gadol.

So we can say that she held like the Cheshek Shlomo that I quoted earlier, ??? ????? ???? ????? ???? ?? ?????? ????? ?”? ???? ???? ??? ????? ??? ???? ?????? ????? ???? ????? ???? ???? and leaving to kill the witch was obviously in the category of ?? ????? ?????? ??????? ????? ?? ????? (though according to the Cheshek Shlomo even stam ????? ????? would suffice). And that is why she had to return to the Emerald City
afterwards – ??? ???? ?????? ????? ???? ????? ???? ????. And she apparently held like Moderator 80 that the wizard leaving was akin to dying, hence she was then allowed to leave. This also very nicely answers the Scarecrow’s kashya of why the Good Witch didn’t tell Dorothy how to get home via the Ruby Slippers the first time they met (without having to resort to the witch’s super dochek answer) – Dorothy wasn’t yet allowed to leave.

March 29, 2015 1:32 pm at 1:32 pm #1066976JosephParticipantThe Evil Witch of the West died because a cyclone lifter up the house of Dorothy’s aunt and landed it on the witch, as oomis pointed out. Dorothy being in the house at the time does not bear her any responsibility, even unintentionally.
The Evil Witch of the East died from being unintentionally doused with a bit of water. Water does not normally kill, as Feivel said.
