If somebody isn’t going to report this article in the news, why not clarify exactly what happened, what the issues are etc as can be seen from the comments.
A friend of mine was once involved in a similar story. During the yichud, thr mesader kiddushin realized that the 2 eidim were related. So he called over my friend and the 2 of them went into the yichud room. He very carefully explained to the couple what to do. He had the kalla give the ring back with daas to be makneh it to him, had him pick it up to be koneh, etc. I don’t think there was any shkia issue involved there (though this article doesn’t explain why shkia was mentioned and what was done about it).
I know another story where the mesader kidushin looked over the ksuba the next day and decided it needed to be redone so he had them do the kidushin again to comply with the date on the ksuba.

Often, years later people realize there is a problem with the Kesuba. They simply write a new Kesuba. They don’t do a new Kedushin.
That sounds very strange. The words aren’t M’akev in this situation anyway because they are clearly Asukin B’oso Ha’inyan.
Also, in your second case, the Kiddushin is completely Kosher. The Kesubah is just Passul so you would need to re-write a second Kesubah. There’s no reason at all to redo the Kiddushin. The Kesubah is not M’akev the Kiddushin in the slightest.
I do not know if any problems can arise from pointlessly redoing the Kiddushin, but in both cases it was unnecessary.
