What you are describing is a very common dilemma.
You are already halfway there by being able to itemize your goals.
Some suggestions:
-write your goal in the positive form (e.g., I am on time rather than I won’t be late)
-read your goals right before going to sleep & when you get up
-set a time frame (e.g., I will finish my book report by such and such date)
-keep your list & time frames realistic
-reward yourself as you achieve your goals & pat yourself on the back!
-divide a larger goal into smaller ones
-have an accountability partner
-expect ups and downs, but keep on going!
-keep your list handy
