Just write the pertinent information on a piece of paper and you’re good.
Trust me— whatever you write can’t be worse than some of the one’s I’ve seen. Just keep it looking neat and clear.
If you want to know what to PUT ON, that’s a different story altogether. There it’s pretty much whatever you want, though there are basic guidelines like name (obviously), age, birthday, personal description (physical and character, can be short), schools attended (high, sem/ yeshiva gedola and college), shul, brief parent info, what you’re looking for, and a reference or two. That’s pretty much bare-bones. You can really put whatever you want on.
You may also need a pic if you’re a girl, unfortunately. Don’t supply unless it’s requested. Many people won’t request it, so why send it if you can avoid it?

its like what writersoul said- your name, your age, schools you went to, camps optional, what your parents do, your shul and some references. mines 1 paper long. 1 shadchan asked for a pic but had my father send it over asking her not to send it out, it is for her reference only.
i did not put a paragraph of what i’m looking for. the ppl who have my resume either know me well or asked me what i want. kinda hard to put it down in words that e/o will get it

Shidduch profile is marginally better IMO, but I don’t much like it, either. Why can’t we just give people information about the boy or girl without labeling it as a businesslike thing?

I usually call it a “sheet of paper of digital document detailing the person’s personal information relevant to dating”. Or “shidduch profile” for short.
(Also, apologies for my brother.)
