    Which Gedolei Poskim are generally reachable for very serious or grave shailos (like pekuach nefesh or other issues), when it requires more than any LOR? Some of the poskim of this caliber would be Rav Feivel Cohen, Rav Yechezkel Roth, Rav Hillel David, Rav Yisroel Belsky, etc. (although I don’t know if they can easily be reached via telephone.)

    Why are you leapfrogging over your own Rav?

    If you feel that your Rav is incapable of answering these questions AND unwilling to admit that he is not knowledgeable enough and is not willing to go to (or refer you to) a greater Rav, then you need a new Rav.

    However, you should be starting with your own Rav — not skipping by him.

    Im not sure about the rabbanim you have mentioned here, however I know that with r’ kemminetzky, you have to go into see him he doesnt have a telephone in his house. Although you can speak with one of the sons or daughters over the phone and they pass on the message.

    Sometimes you know yourself that your regular Rov will not pasken a major shaila, so you go directly to a great posek. An example might be whether to r”l pull the plug on a ventilator.

    Ca, I thought he doesn’t talk on the phone at all. I guess if you’ve done it personally, I’ve got a new way how to speak to him directly, thanx for that!

    If you know *for sure* that your Rav will send you to Gadol X for a question of this type, then fine. But the OP clearly does not know that if he is asking the question.

    That being said, if he does not trust his Rav to pass the question on (or refer him) to someone qualified, then he needs a new Rav. He should not be asking strangers who is qualified to make a major life-and-death decision.

    I’m not sure where you got that R’ Shmuel does not have a phone – I was in his house and saw him talking on the phone.

    I have heard directly from the Gadol haDor of last decade:- Rav Avigdor haKohen Miller ZTKLLH’H:- Your Rov {Asseh leCho Rov} must be alive & acessible.

    If he ain’t easily accesible, his Rabbonus in your life is worthless. Get an accesible Rov. If your accesible Rov feels it is beyond him, he’ll have access to higher up, becaue of his position.
